Public health, primary care, and community partners will converge in Washington, DC from May 31 to June 2, 2017 for the 2017 Practical Playbook National Meeting at the Westin Washington, DC City Center. The Practical Playbook National Meeting is a forum for national dialogue on best practices for cross-sector collaborations in population health, while rising to meet the challenges of the changing healthcare landscape. This year’s theme is “Improving Population Health: Collaborative Strategies that Work.” Experts from the Public Health Foundation (PHF) will be leading a breakout session on change leadership during this year’s event and looking to connect with event attendees who are eager to learn more about our expertise in improving population health, and our focus areas: performance management, quality improvement, and workforce development.

Related to the theme of this year's meeting, earlier this year, PHF released a new book, Solving Population Health Problems through Collaboration, which provides valuable and practical insights from over 50 experts in population health. The facilitators of PHF's breakout session at the Practical Playbook National Meeting, Ron Bialek, PHF's President, and Jack Moran, PHF's Senior Quality Advisor, edited this timely publication. These critical behaviors and activities will be defined within the context of a Plan-Do-Check-Act framework familiar to practitioners of quality improvement. Lessons learned will be applicable to public health agencies, healthcare organization, and others supporting efforts of public health and healthcare.
