The Fall 2016 Open Forum for Quality Improvement in Public Health (Open Forum) will be held October 6-7, 2016 in Salt Lake City, Utah. The Open Forum facilitates the exchange of best practices, expands the community of practice, and builds capacity among public health departments for accreditation and quality improvement (QI) activities. The event focuses on contemporary advances, inspirational stories, tools, and cutting edge topics from the field of QI in public health. The Open Forum is hosted by the National Network of Public Health Institutes and is sponsored by the Robert Wood Johnson Foundation.
 
Public Health Foundation (PHF) experts Jack Moran and Sonja Armbruster will lead two breakout workshops at this year’s Fall Open Forum, to increase attendees’ knowledge of using QI in practice:

"How to Get the CHIP Off the Shelf and Into Practice"
Participants in this session will learn how to apply three quality improvement tools to help make their Community Health Improvement Plan (CHIP) a community health improvement practice – not just a plan. This interactive workshop will explore why health departments have problems getting the CHIP into practice. A diagnostic force field analysis will be used to help participants better understand the driving and restraining forces that contribute to implementation of the CHIP not getting off the ground. Participants will then use a cause and effect diagram to understand the internal and external root causes that contribute to CHIP inaction. Then participants will use a solution and effect diagram to determine how to move the CHIP from a plan to a dynamic tool for action to improve community health. Finally, a small group discussion will be held on what it will take for the community to understand, engage with, and utilize the CHIP to improve their community’s health.
 
This session will draw upon content used in PHF's on-site service, Implementing Health Improvement Solutions, which can be customized for use with any health challenge in any community. 

"How to Use Highly Effective Teams to Support Performance Improvement Efforts"
This interactive workshop will help the participants understand the following:
  • Why teams fail
  • Techniques to build and maintain high-performing teams
  • Team members’ preferences
  • Team members’ roles and responsibilities
  • Characteristics of effective teams
  • Team performance cycles
  • How to get a team moving again
  • How to make teams successful by selecting the right team members
Participants will develop a diagnostic force field analysis to understand the driving and restraining forces connected to implementing effective teams in a health department. In small groups, participants will utilize a personal preference instrument to understand how conflict develops within teams and how to anticipate and overcome conflicts by selecting appropriate team members in advance. Then participants will develop a cause and effect diagram to identify the root causes of health department staff resisting team involvement.
 
This session will draw upon content used in PHF's on-site service, Developing High-performance Teams, which can be customized for use in any community.

Fall 2016 Open Forum for Quality Improvement in Public Health
